CurMap 2025

Mapped island-wide for the first time: six years of community pharmacy dispensing (SVB, about 90% of the population) and all inpatient use at Curaçao Medical Center, set against the Dutch NethMap figures.

Zes kernboodschappenSix key messages

Use outside the hospital is rising while hospital use is falling. Together these two trends shape the selection pressure on resistant bacteria on the island.

Carbapenems: de daling is omgebogenCarbapenems: the decline has reversed

Between 2021 and 2024 carbapenem use fell from 5.94 to 3.33 DDD/100 patient-days. In 2025 it rose by 0.66 to 3.99, more than twice the Dutch level (1.67). Carbapenems are essential for severe infections with multidrug-resistant Gram-negative bacteria. Possible explanations include changing resistance patterns, more complex infections or shifts in empirical prescribing. Targeted evaluation is recommended.

Access, Watch en ReserveAccess, Watch and Reserve

WHO groups antibiotics by resistance risk. The European Union's 2030 target is that at least 65% of total use comes from the Access group.

Access

Eerste keus, smal spectrum, laag resistentierisico. Bijvoorbeeld amoxicilline, nitrofurantoïne.First choice, narrow spectrum, low resistance risk. For example amoxicillin, nitrofurantoin.

Watch

Hoger resistentierisico, alleen bij specifieke indicaties. Bijvoorbeeld macroliden, fluorochinolonen.Higher resistance risk, for specific indications only. For example macrolides, fluoroquinolones.

Reserve

Laatste redmiddel. Last resort. At CMC in 2025: tigecycline, ceftazidime-avibactam and colistin.

Methode & definitiesMethods & definitions

Hoe is er gemeten?How was this measured?

CurMap follows the WHO ATC/DDD methodology, like NethMap. That makes the figures directly comparable with the Netherlands and internationally.

Wat is een DDD?What is a DDD?

The Defined Daily Dose is the WHO-assigned average adult maintenance dose per day for the main indication. It lets agents with different dosages be added up and compared.

DDD's tonen volume, niet het aantal voorschriften of behandelde patiënten.DDDs show volume, not the number of prescriptions or treated patients.

Eerstelijn: DDD per 1.000 inwoners per dag (DID)Outpatients: DDD per 1,000 inhabitants per day (DID)
DID = (Σ DDD / aantal SVB-verzekerdennumber of SVB insured / 365) × 1.000

Alleen J01 (systemisch).Data: community pharmacy dispensing to SVB insured, about 90% of Curaçao's population. Includes prescriptions by GPs, medical specialists and others such as dentists, and dispensing to nursing-home residents via community pharmacies. J01 (systemic) only.

Ziekenhuis: DDD per 100 patiëntdagenHospital: DDD per 100 patient-days
patiëntdagen = beddagen − opnamespatient-days = bed-days − admissions

Bed-days count both admission and discharge day as full days, so admissions are subtracted. Data from HiX (CMC), DDDs per ATC code and route from the Dutch Z-Index.

Wat zit er niet in de data?What is not in the data?

The indication, the number of patients, non-SVB insured (about 10%), veterinary use and resistance data. Outpatient use is therefore not the same as GP use.

Waarmee wordt vergeleken?What is the comparison?

The Dutch reference figures from NethMap (SWAB/RIVM), measured with the same methodology. Changes and percentages were calculated from unrounded values.
